Artemis Healthcare Services LLC provides comprehensive in-home healthcare services, including but not limited to:

- Post-surgical and post-hospitalization care

All services are provided in accordance with applicable state and federal regulations, professional standards of care, and physician orders where required.
3. Eligibility and Admission
To receive our home health care services, patients must:
- Reside within our service area in Greenwood Village, Colorado and surrounding communities.
- Have a safe and suitable home environment for the delivery of care.
- Provide or arrange for a physician's order for skilled services when required.
- Consent to our assessment and care planning process.
- Agree to participate actively in their care plan and follow medical recommendations.
We reserve the right to decline or discontinue services if the home environment is deemed unsafe for our staff, if patient or family behavior poses a threat to staff safety, or if medical needs exceed our scope of practice.
4. Care Planning and Coordination
Upon admission to our services:
- A comprehensive assessment will be conducted by a qualified healthcare professional.
- An individualized plan of care will be developed in collaboration with the patient, family, and physician.
- The care plan will be reviewed and updated regularly based on patient progress and changing needs.

5. Patient Rights and Responsibilities

Patients receiving our services have the right to:

6. Scheduling and Cancellations
Scheduling:
- Service visits will be scheduled based on the care plan, patient needs, and staff availability.
- We will make reasonable efforts to accommodate patient scheduling preferences.
- Visit schedules may need to be adjusted due to emergencies, staff illness, or other unforeseen circumstances.
- We will provide advance notice of schedule changes whenever possible.
Cancellations and No-Shows:
- If you need to cancel or reschedule a visit, please provide at least 24 hours advance notice.
- Cancellations with less than 24 hours notice may limit our ability to reschedule promptly.
- Repeated no-shows or last-minute cancellations without valid reason may result in discharge from services.
- We understand that emergencies occur and will work with patients to accommodate legitimate urgent situations.
7. Payment, Billing, and Insurance

- Patient moves outside our service area.
- Patient is hospitalized or admitted to a facility for an extended period.
- Patient or family behavior poses a safety risk to our staff.
- Patient consistently refuses to cooperate with the care plan.
- Home environment is deemed unsafe for service delivery.
- Patient fails to meet financial obligations after appropriate notice and opportunity to resolve.
Whenever possible, we will provide advance written notice of discharge and assist with transition planning to ensure continuity of care.
9. Confidentiality and HIPAA Compliance
Artemis Healthcare Services LLC is committed to protecting patient privacy and complying with the Health Insurance Portability and Accountability Act (HIPAA):
- All patient health information is kept strictly confidential and secure.
- Information is only disclosed as necessary for treatment, payment, healthcare operations, or as required by law.
- Patients will receive a Notice of Privacy Practices explaining how their information may be used and disclosed.
- Patients have the right to access their medical records and request corrections or amendments.

- Smoking in the home during visits is prohibited due to health and safety concerns.
12. Limitations of Services
Please be aware that:
- Home health care services are provided on an intermittent basis, not 24/7 continuous care.
- We do not provide emergency medical services; patients should call 911 for medical emergencies.
- Our services supplement, but do not replace, the care and supervision provided by family members or caregivers.
- We are not responsible for patient care, safety, or supervision when our staff is not present in the home.
- Certain medical procedures or conditions may be outside our scope of practice and require hospitalization or specialized care.
13. Limitation of Liability
To the fullest extent permitted by law:
- Artemis Healthcare Services LLC shall not be liable for patient outcomes related to underlying medical conditions, only for the care provided by our staff within their scope of practice.
- We are not responsible for injuries or incidents that occur when our staff is not present in the home.
- Our liability for any claims arising from our services shall be limited to the amount paid for the specific service giving rise to the claim.
- We maintain appropriate professional liability insurance as required by state regulations.
Important: This limitation does not apply to cases of gross negligence or willful misconduct by our personnel.
14. Quality Assurance and Complaints
We are committed to providing high-quality care and addressing concerns promptly:
- We conduct regular quality assurance reviews and patient satisfaction surveys.
- Patients and families are encouraged to provide feedback about our services.
- Complaints or concerns should be reported to our office immediately so we can investigate and address them.

15. Governing Law and Dispute Resolution
These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Colorado, without regard to its conflict of law provisions.
- Any disputes arising from these terms or our services shall be resolved through good faith negotiation or mediation before pursuing legal action.
- If mediation is unsuccessful, disputes may be resolved through binding arbitration in Greenwood Village, Colorado, except where prohibited by law.
- Each party shall bear their own costs and attorney's fees unless otherwise awarded by the arbitrator or court.
